Are you relying on OSHA LOTO exceptions to simplify compliance?
The Cord & Plug and 8 Criteria procedure creation exemptions are narrower — and riskier — than most organizations realize.
⚠️ The High Stakes of Hazardous Energy Control
LOTO is not a paperwork exercise. OSHA 29 CFR 1910.147 exists to prevent unexpected energization or release of stored energy during servicing and maintenance.
Hazardous energy sources include but not limited to:
- ⚡ Electrical Energy
- 💧 Hydraulic Energy
- 💨 Pneumatic (Air) Energy
- 🌊 Water Energy
- ⬇️ Potential Energy (Gravity, Springs, Capacitors)
- 🔄 Kinetic Energy
- 🧪 Chemical Energy
- 🔥 Thermal & Steam Energy
🔌 The Cord and Plug Exception: A Fragile ‘Total Exemption’
The Cord and Plug Exception (1910.147(a)(2)(iii)(A)) is the only true total exemption from the LOTO standard.
Qualification requires:
- 1️⃣ Plug-powered equipment only.
- 2️⃣ Unplugging eliminates ALL hazards of unexpected energization.
- 3️⃣ The plug remains under exclusive control of the employee.
🚨 The moment the employee walks away from the plug — the exemption is void.
⚡ The Multi-Source Trap
If equipment has more than one energy source, unplugging does NOT make it safe.
- 🔋 Battery backups
- 🔌 Capacitors
- 💨 Pneumatic air lines
- 💧 Hydraulic pressure
- 🌀 Stored spring tension
- ⬇️ Gravity hazards
- 🔄 Residual kinetic energy
❗ If a secondary energy source exists, the exception is immediately nullified.
Full lockout procedures and machine-specific documentation are required.
📋 The 8 Criteria Exception: Paperwork Reprieve — Not Lockout Reprieve
This exception removes the written procedure requirement only if ALL eight conditions are met.
- ✔️ Zero residual energy after shutdown.
- ✔️ Only one identifiable energy source.
- ✔️ Complete deactivation from a single lock.
- ✔️ Machine locked out during entire service.
- ✔️ One lockout device achieves full isolation.
- ✔️ Exclusive control of the key.
- ✔️ No hazards to other employees.
- ✔️ Clean safety record (no activation incidents).
Fail one criterion — and a written procedure is mandatory.
🎯 The Strategic Imperative: Lockout First
Exceptions introduce ambiguity. Ambiguity introduces risk.
The safest and most defensible strategy is simple:
🔒 When in doubt — lock it out and write it down.
- 🛡️ Life-saving protection
- ⚖️ Legal safeguard
- 📈 Operational consistency
- 🏗️ Stronger safety culture
